Home Health Physical Therapist (Contract)
Platinum Medical Staffing is seeking licensed Home Health Physical Therapists to join our team under a continuous contract to provide in-home therapy services throughout Northern Virginia. As a contracted clinician, you'll manage a consistent caseload, delivering high-quality care to patients in the comfort of their own homes.
This ongoing opportunity offers flexibility, competitive per-visit pay, and dedicated support from our experienced administrative team.
Key Responsibilities:
- Deliver in-home physical therapy in accordance with physician orders
- Complete SOC, ROC, evaluations, follow-up visits, and recertifications as required
- Conduct comprehensive fall risk and functional mobility assessments
- Create and implement individualized Plans of Care (POC)
- Document all care accurately in Homecare Homebase within 24 hours
- Maintain regular communication with skilled nursing and care coordination teams
- Confirm appointments with patients and promptly report any schedule updates
Requirements:
- Active Physical Therapist license in Virginia
- Valid CPR certification
- Familiarity with OASIS-E documentation standards
- Experience using Homecare Homebase preferred, or willingness to learn
- Excellent time management and communication skills
- Reliable transportation and willingness to travel locally
