The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team.No on-call, no weekends, and no holidays required. As one of the largest employers of APCs, Optum offers unparalleled career development opportunities.Primary Responsibilities:Conduct comprehensive assessmentsManage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventionsManage members during a short term sub-acute rehabilitation episodeEnsure accurate and complete documentation, including ICD 10 conditionsCommunicate and collaborate with the interdisciplinary care teamConduct advanced illness and advanced care planning conversationsProvide patients and caregivers with counseling and educationYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:For Nurse Practitioners:Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice programActive and unrestricted Nurse Practitioner license in the state which you reside, or ability to obtain by start dateBoard certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:Family Nurse PractitionerAdult Nurse PractitionerGerontology Nurse PractitionerAdult-Gerontology Acute Care Nurse PractitionerAdult-Gerontology Primary Care Nurse PractitionerFor Physician Assistants:Graduate of an accredited Physician Assistant degree programActive and unrestricted Physician Assistant license in the state which you reside, or ability to obtain by start dateBoard certified by the National Commission on Certification of Physician Assistants (NCCPA)Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice) (SCC04)Ability to gain a collaborative practice agreement, if applicable in your stateAbility to complete physical requirements of the job including moving a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, remain in a stationary position and position self to perform physical assessmentDriver's license and access to reliable transportationPreferred Qualifications:1+ years of clinical experience in practice (long-term care setting preferred)Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needsUnderstanding of Geriatrics and Chronic IllnessUnderstanding of Advanced Illness and end of life discussionsProficient computer skills including the ability to document medical information with written and electronic medical recordsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,900 to $160,600 annually based on full-time employment.
