Who You Are:
We are seeking a candidate with a strong positive attitude toward customer service to act as a Social Worker. This person will have a genuine interest in working with older adults and be able to engage, motivate, and support both residents and fellow team members. Essential Functions

1. Being available to welcome the new resident, provide support and initiate a trusting relationship.

1. Entering an initial assessment note in the Social Service section of the chart within 72

hours of admission, completing assigned sections of the MDS, completing and entering


the Social History within fourteen (14) days of admission and re-evaluating the resident on a quarterly basis (sooner when a significant change occurs). These assessments are used to contribute to the resident care plan.

1. Contributing as an integral member of the interdisciplinary Resident Care Plan

Conference. This also involves stating the social/emotional needs related to the resident's illness/disability, adjustment to placement, supportive network and response to treatment. The social worker makes specific recommendations to assist in the resident's overall care and genuine well-being.

1. Coordinate weekly care plans meetings and notify resident(s), family, IDT members.

1. Writing progress notes as least on admission quarterly, earlier when there is a notable change. Each note should document progress made towards "social service" oriented goals, provide significant information about the individual and serve to communicate with the entire interdisciplinary team.

1. Initiating and/or participating in the written discharge plan which states the resident's

specific need to be in the facility and if the resident is expected to be able to function in a more independent setting. The discharge plan should include consultation with other


disciplines, the family and of course, the resident (whenever possible).

1. Participating in the post D/C plan as needed or assigned.

1. Provide "active listening" therapy with residents who express the need to talk with a

supportive individual. The social worker must work on establishing a good rapport with


residents and be available to provide behavioral intervention/counseling.

1. Attending Resident Council meetings as a liaison to listen to resident concerns and clarify issues and/or facilitate changes (when invited by the Resident Council governing body). The Social Worker should be involved with the Activity Department and assist with the Resident Council meetings as necessary.

1. Contacting community agencies for the purpose of resident referral. These agencies include, but are not limited to: the Department of Mental Health and Developmental Disabilities, Department of Public Aid, Veteran's Administration, community mental health centers, transportation companies, social clubs, etc.

1. Assisting the resident to participate in individual and group programs and to utilize these programs to full advantage.

1. Educate residents/families and assist with the completion of advance directives.

1. Be available to assist residents, with social issues, in Assisted and Independent settings.

1. The social worker is a resident advocate, as well as a liaison between the resident and his/her family, the facility and community agencies.

1. The social worker will provide family educational/support services, as required.

1. The social worker should also be available to meet with nursing consultant as needed or assigned and available to assist in the planning and implementation of additional programs (i.e. family nights, open house, and joint programs with the Activity Department).

1. The social worker should promote resident rights issues, including: self-determination, treatment of resident with respect and dignity, the right to voice grievances, and the right to meet with other residents.

1. Help plan, implement and organize the development of the Family Council/Family Support Group (as needed).

1. The Social Worker will be instrumental in providing Quality Assurance services. This may be accomplished with formal and informal QA studies/audits.

1. Providing emotional support and adjustment counseling as an adjunct service to programs presently in place within the facility with the supervisor's permission and guidance.

1. Social worker will implement/monitor/coordinate behavioral health meetings monthly with IDT and at least quarterly with physicians involvement.
2. Maintain work area and keep the department in compliance with OSHA safety standards.
3. Effectively communicates both verbally and in writing.
4. Demonstrates the community hospitality promises towards residents, families, associates and vendors.
5. Is "on-call" status during time away from work, such as after working hours or during days off. Also during critical operating issues and during disaster emergencies such as: snow, ice storms, fire, tornados, floods or as needed by the CEO.
6. Serves as the primary Grievance Official in the Health Center. Duties of the Grievance Official include:
   a. Overseeing the grievance process.
   b. Receiving and tracking grievances through to their conclusion.
   c. Leading any necessary investigations by the facility.
   d. Maintaining the confidentiality of all information associated with grievances, for example, the identity of a resident.
   e. Preventing further violations while under investigation.
   f. Documenting.
   g. Meeting all applicable State and Federal laws and regulations.

1. Must be able to read, write and follow written and verbal directions.
2. Demonstrates skills and ability in working with and understanding the needs of theresidents, families, other staff members and personnel from community agencies.
3. The ability to present concise, meaningful written reports and articulate the social/psychological needs of the residents.
4. The ability to accept and utilize professional supervision, consultation and in-service training.
5. The ability to handle confidential data with professional discretion.
6. Experience in long term care setting.
