Job Summary Utilize evidence-based medical nutrition therapy to assess and reassess nutrition risk and identify nutrition needs. Develop and implement nutrition care plans to support oral and/or nutrition support needs. Participate in quality improvement initiatives/committees. Provide nutrition counseling to patients and families as warranted. Participate in multidisciplinary interaction and education. Primarily working in dedicated area of expertise, supported by board certification in a nutrition specialty. Responsibilities and Duties Perform nutrition assessment using age-appropriate standards, utilizing validated assessment tools and nutrition focused physical assessments; include documentation of nutrition related diagnosis when indicated. Demonstrate proficiency in accordance with Standards of Practice and Professional Performance for Registered Dietitian Nutritionist while providing age-appropriate plan of care for patients and families. Coordinate nutrition therapy with physicians and other health care professionals. Participate in multidisciplinary rounds and care planning sessions. Plan, implement, and adapt nutritional care plan to meet physician orders, age and disease specific physiologic needs; intervention plan includes appropriate feeding route, mode, and nutrition prescription. Perform tasks required by feeding tube consults including placing, repositioning, and replacing tubes, inserting, and repairing nasal retainment device, and other duties related to tube insertion and maintenance, as applicable. Monitor and evaluate patient response to medical nutrition therapy using specific goals and determine changes if indicated. Perform patient, family, and caregiver education related to nutritional therapy and incorporate best method of learning style and communication into sessions. Educate and train interdisciplinary team members, students, and interns. Foster teamwork and establish effective strategies to implement innovative programs which lead to optimal clinical nutrition and financial outcomes. Perform quality improvement, research, and outcome studies. Identify and communicate opportunities for waste reduction and service improvements. Document accurately, completely, and appropriately in electronic medical record. Assist in the development and revision of nutrition related department tools, training materials, policies, and procedures. Participate in the evaluation of clinical nutrition products and equipment. Comply with all regulatory standards, including federal, state, and accrediting agencies and adhere to facility confidentiality, HIPAA regulations, and patient rights policies. Participate in and attend departmental meetings, staff development, and professional programs, as appropriate. Serve as an active member on multidisciplinary teams. Ensure national program standards are met and always maintained for certified and/or recognized programs, as applicable. Perform tasks associated with the assessment, evaluation, delivery, and maintenance of parenteral nutrition, as applicable. Perform tasks associated with educating, supporting, and advocating for patients and clients to understand and manage their weight status and associated risks using nutritional, behavioral health, medical, surgical, pharmacotherapeutic, and exercise and physical activity interventions, as applicable. Perform tasks associated with the collaboration and education of patient/family members, and medical team members to better manage diabetes, while minimizing short and long‑term diabetes complications, as applicable. Perform tasks associated with cardiac rehabilitation competencies in accordance with the CCRP Exam Blueprint and the CCRP Candidate Handbook, as applicable. Perform tasks associated with direct interactions with individuals at risk for, or diagnosed with, any type of malignancy or pre‑malignant condition, in a variety of settings, as applicable. As a High Reliability Organization (HRO), responsibilities require focus on safety, quality and efficiency in performing job duties. Minimum Qualifications Bachelor's Degree (Required) Ohio Board of Dietetics Licensure - Ohio Dietetics Board Certification obtained from the following nationally recognized organizations (other certifications may be reviewed on an individualized basis): National Board for Nutrition Support Certification Commission on Dietetic Registration Certification Board for Diabetes Care and Education American Association of Cardiovascular and Pulmonary Rehabilitation The Academy of Lactation Policy and Practice, Inc.
