Key Responsibilities

• Dispense and verify outpatient prescriptions accurately and in compliance with applicable regulations
• Perform drug utilization reviews and monitor for drug interactions, contraindications, and dosing appropriateness
• Collaborate with clinical staff and providers to optimize patient medication therapy
• Counsel patients on proper medication use, side effects, and adherence
• Maintain accurate patient medication records and documentation

Required Qualifications

• Doctor of Pharmacy (Pharm.D.) degree or equivalent from an accredited institution
• Active and unrestricted Pharmacist license in the state of New York
• Experience in an outpatient or retail pharmacy setting
• Strong knowledge of pharmaceutical regulations and dispensing procedures

Preferred Experience / Skills

• Experience working within a health system or hospital-affiliated outpatient pharmacy
• Familiarity with electronic health record (EHR) and pharmacy management systems
• Strong patient communication and counseling skills
