Job Description

Primary Accountability

Responsible for the provision of optometry services in a Patient Centered Health Home context with focus on access to care.

Description of Primary Responsibilities:
  1. Responsible for the direct provision of optometry services meeting all established productivity expectations.
    1. Provides eye examinations, using observation, instruments, and pharmaceutical agents to determine visual acuity and perception, focus and coordination, and to diagnose disease and other abnormalities within the scope of optometry.
    2. Analyzes test results and develops a treatment plan.
    3. Prescribes, supplies, fits and adjusts eyeglasses, contact lenses, and other vision aids.
    4. Prescribes medications to treat eye diseases, with proper licensure and abiding to state law.
    5. Removes foreign bodies from eye, following state law guidelines.
    6. Follows-up on specific disease categories and no show evaluations.
    7. Reviews laboratory, x-ray, and referral documents prior to filing.
    8. Documents visits accurately utilizing appropriate templates, and records relevant data including but not limited to acute and chronic problems.
    9. Attains budgeted visits.
    10. Meets clinical care measures.
    11. Locks records in the 72-hour window.
  2. Accountable for providing excellent customer service and care.
    1. Consistently behaves with courtesy when interacting with patients/family members, support staff, and Network staff.
    2. Provider remains aware of wait time during the daily workflow.
    3. Responsible for providing the patient with educational materials and resources appropriate to the patient's health literacy needs.
  3. Accountable for patient status as measured by relevant clinical quality measures.
    1. Promotes and participates in monthly meetings and huddles.
    2. Incorporates the participation of ancillary health team members in managing the care of patients/families.
    3. Responsible for setting and monitoring self- management plans.
    4. Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.

    7. Completes peer review responsibility in a timely manner.
  4. Assists supervisors in planning and implementing new programs for the optometry department.
    1. Participates in Network Health Fairs, screening activities and other community outreach events.
    2. Where relevant assist the optometry director with the review and update of clinical policies and procedures.
    3. Participate in site and provider meetings in a meaningful way.
    4. Responsible for participating with sick coverage, per schedule.
    5. Meets Network and credentialing expectations for licensure, and employment.

  6. Responsible for adhering to the Attendance and Absenteeism Policy, recognizing that regular attendance is considered an essential function of all FHCN positions. Absenteeism is not being at work or failing to attend a paid workshop, training, or event unless the absence is protected by law.
  7. Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  8. Performs other duties as assigned.
Description of Primary Attributes

Professional & Technical Knowledge:

Effective June 1, 2026, all individuals hired into the role must:
  1. Possesses specific advanced knowledge and skills, including written and verbal communication skills, computational, computer, and technical skills, and mathematical knowledge, frequently acquired through one of the following:
  2. Completion of a Doctoral-level Degree program with a recognized major and a minimum cumulative GPA of 2.5; or
  3. Successful attainment and maintenance of a valid, active professional license required for the role, issued by the appropriate governing or regulatory body.
  4. Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training in Optometry studies, including an understanding of the application of the theory and practices of the profession to the operations of the organization, usually in the form of a Doctor of Optometry degree.
Technical Skills:
  1. Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.
  2. Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.
  3. Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions and other elements.
Licenses & Certifications: Licensed in the state of California as an Optometrist.


Communications Skills:
  1. Requires the employee to effectively communicate their professional opinions and extrapolations of information they collect and synthesize/analyze.
  2. Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts and other visual forms.
  3. Duties require the preparation and execution of presentations to large groups.

While performing the duties of this position, the employee is regularly required to sit and use repetitive hand movement to type and grasp. The employee is frequently required to stand or walk; and the employee must occasionally lift and/or move up to 20 pounds.

$150,085.63 - $240,137.00 + PLUS SIGNIFICANT ADDITIONAL ANNUAL EARNING POTENTIAL through outpatient productivity and quality pay, generous retention payments, shift differential pay, and extra shift pay. Sign-on bonus/relocation assistance, annual CME days and reimbursement also available. Generous health and wellness benefits and retirement packages offered.
