Job Description

The Certified Diabetes Care and Education Specialist (CDCES) will provide diabetes education, consultation, and evaluation of patient and program outcomes in the outpatient Diabetes Education Program. The role will involve work with diverse patient populations, including adults, geriatrics, and pediatrics, encompassing those with type 1 and type 2 diabetes as well as gestational diabetes.

Responsibilities

• Deliver educational materials using teaching principles and various formats for new pediatric inpatients and outpatients.
• Communicate and implement teaching interventions for patients, families, and significant others.
• Assist in maintaining diabetes supplies and provide support to other staff as necessary.
• Participate in meetings and assist in orientation and training of personnel and students.

Required Experience

• Experience in Diabetes Self-Management Education & Support (DSMES).
• Background in insulin education, continuous glucose monitoring (CGM) education, counseling, and care coordination.

Certifications/Licensure

• Certified Diabetes Care and Education Specialist (CDCES).
• Registered Dietitian Nutritionist (RDN) or Registered Nurse (RN) license in the State of Michigan.
• Basic Life Support Healthcare Provider (BLS-HCP) certification.
