Hospice RN Case Manager | Patient-Centered Care

JourneyCare Hospice is seeking a skilled RN Case Manager to join our growing care team in McHenry County, IL. This role oversees patient and family caseload, collaborating with interdisciplinary staff to deliver comprehensive hospice care. The position requires an accredited RN program, active RN license, CPR/First Aid, and a valid driver’s license. Hours are Mon–Fri 8:30am–5:00pm with on-call every 12th weekend, in a supportive culture with extensive benefits. #J-18808-Ljbffr JourneyCare
