**Join Our Dynamic Team as a Physical Therapist - Home Health!** **Job Description:** - Are you passionate about helping others regain their independence and improve their quality of life? - We are looking for a Physical Therapist to join our innovative home health team in the vibrant San Mateo area! **Key Responsibilities:** - Restore function and minimize disability for patients recovering from disease or injury. - Evaluate and treat patients using medically prescribed physical therapy programs. - Supervise Physical Therapy Assistants as needed. - Conduct initial admissions and evaluation visits with compassion and expertise. - Notify the assigned Clinical Team Manager of OASIS scores within established timelines. - Document observations, clinical findings, skilled interventions, goals, and discharge plans with precision. - Embrace other duties as assigned, aligning with our goals and mission. **Territory Details:** - Cover a diverse and vibrant territory including: - South San Francisco - Pacifica - Millbrae - Hillsborough - Burlingame - San Carlos - Foster City - San Mateo - Redwood City - Emerald Hills - Menlo Park - Mountain View - Neighboring cities as needed **Requirements:** - Minimum of one year of Home Health experience. - Certifications: BLS/BCLS. **Shift Information:** - Days: 8:00 AM - 5:30 PM (With flexibility based on patient availability). **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
