Description
American Traveler is seeking a licensed Physical Therapist with Home Health experience for a 13-week travel contract in Anchorage, AK, requiring HCHB and OASIS proficiency.
Details
- Home Health setting serving a geriatric patient population
- Territory covers up to a 60-mile radius from the hub/office
- Day shift schedule, 5x8 hours (8:00–17:00)
- Caseload of 5–7 patients per day, with a minimum of 6 visits daily or 30 points weekly
- Uses HCHB (HomeCare HomeBase) as the EMR
- No on-call requirement
- Local candidates are accepted at the travel rate
- iPad provided for use in the field; mileage reimbursed at the IRS standard rate
Requirements
- Current PT license required; license in hand required if applicable
- Minimum 1 year of recent Home Health physical therapy experience required
- OASIS experience required
- Experience with HCHB EMR required
- Current BLS certification required
- Ability to manage a defined geographic territory of up to 60 miles from the home office
Additional Information
- Timely communication and thorough documentation are essential in this role
- Scrubs required in any color; jeans are not permitted
- All time-off requests must be submitted at the time of initial consideration
- 2 professional references from a supervisor within the past 2 years required for consideration
- Full 7-year work history with all gaps explained required for consideration
