Responsibilities Conduct initial patient assessments, develop personalized care plans, and provide skilled nursing care to patients in their homes Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure comprehensive care Administer medications, perform wound care, and provide education to patients and their caregivers Monitor patients' progress, report changes, and make recommendations for adjustments to care plans Maintain accurate and detailed records of patient interactions, treatments, and progress Requirements Current registration with IDFPR as a Registered Nurse (RN) Valid Driver's License (DL) and reliable transportation Experience:Home health, acute care or skilled nursing preferred Level of Education: ADN or BSN Ability to drive to patient home across assigned territory. Ability to work in varied home environments Proficiency with EMR systems.
