Physical Therapist (AHCL)

Day Shift; M - F ; 8am to 5pm; outpatient PT position, minimal acute inpatient coverage from time to time.Full-time, Part-time, Contract, Temporary

Responsibilities

• Assess and treat patients with mobility, pain, and function impairments.

• Develop individualized PT plans with measurable short- and long-term goals.

• Monitor patient progress and adjust therapy plans accordingly.

• Document and maintain accurate patient records and outcomes.

• Educate patients and families on home exercises and injury prevention.

• Collaborate with multidisciplinary team to coordinate care.

• Utilize evidence-based practices to inform treatment decisions.

• Ensure compliance with safety, ethical, and regulatory standards.

• Meet performance targets and contribute to pipeline-based planning.

• Demonstrate proactive professional development and skill advancement.

• Participate in on-site clinical rounds and case reviews.

• Engage in performance reviews and contribute to merit-based rewards.

Experience/Qualifications

• Licensed Physical Therapist with current state license.

• Demonstrated ability to Assess, diagnose, and treat mobility and pain impairments.

• Develop and implement individualized PT plans with measurable goals.

• Accurate documentation of patient records and outcomes in EMR systems.

• Apply evidence-based practices to inform treatment decisions.

• Adhere to safety, ethics, and regulatory standards in practice.

• Commit to ongoing professional development and skill advancement.

• Educate patients and families on home exercise programs and injury prevention.

• Collaborate with multidisciplinary team for coordinated care delivery.

• Meet performance targets and contribute to pipeline-based planning.

• Willingness to work on-site in a growth-oriented, merit-driven environment.

• Ability to work within a measurement-driven culture with high activity visibility.

• Participate in on-site clinical rounds and case reviews for learning.
