JD:
- 8+ years of project management experience, preferably in healthcare or population health
- Strong experience with Agile methodologies (Scrum Master or Agile certification preferred)
- Experience working with population health management tools and data analytics
- Knowledge of healthcare systems, EMRs/EHRs, and care management workflows
- Proven ability to manage cross-functional teams and complex projects
