Acute Rehabilitation Case Manager

This is a 13-week contract position.

Shift: 08:00 AM - 04:30 PM, Monday to Friday

The Acute Rehabilitation Case Manager will work within a state-of-the-art inpatient rehabilitation facility dedicated to the treatment and recovery of individuals with severe injuries or illnesses. The role is suited for seasoned Case Managers with experience in adult acute hospital settings. The successful candidate will exercise effective communication and collaboration skills to support patients' rehabilitation journeys.

Responsibilities:

• Manage and coordinate patient care for individuals undergoing acute rehabilitation.
• Engage in patient and family conferences to discuss treatment plans and progress.
• Perform utilization review and adhere to acute rehabilitation criteria.
• Document and maintain accurate patient records in the electronic health record system.
• Collaborate with nursing, therapy, and medical staff to optimize patient outcomes.

Required Experience / Certifications / Licensure:

• Current licensure as a Registered Nurse, Social Worker, or any healthcare professional with case management experience.
• 2-3 years of relevant experience in acute rehabilitation preferred.
• Case management certification is preferred (e.g., CCM, ARN, ACM).
• Strong oral and written communication skills; proficiency in English, additional languages preferred.
• Basic computer skills including proficiency in Excel, Word, and Outlook.
