In this impactful role, you will partner closely with Care Managers to coordinate comprehensive care plans, support professional growth, and drive positive outcomes for the individuals we serve.If you are a passionate leader who thrives in a collaborative environment and is committed to improving the lives of others, we invite you to join our team!Responsibilities:Provides supervisory support to assigned Care Management TeamProvides individual and group supervision to assigned Care Managers and evaluates all supervisees on a regular and timely basisMonitors assigned Care Managers' work activities with children and families through team meetings and regular individual meetingsReviews and approves encounter notes, CANS assessments, Comprehensive assessments and Plans of Care for accuracy and compliance.Reviews required documentation contained required in the Electronic Health Record..Ensures Care Managers complete required trainings.Provides direct care management services in the absence of Care Management staffManages, reviews, and monitors maintenance of member records to ensure both internal and external quality assurance standards and timely completion in accordance with agency and CCF Health Home PoliciesWorks closely with QA team members to insure audits are accurate by responding within 5 business days of any needed revisionsProvides training to new staff and on-going training to existing staffDemonstrates strong leadership and management skills to ensure progressive, positive development of Care Management employeesCoordinates and conducts plans for audit preparation and audit execution.Requirements:Master's Degree in Social Work, Nursing, Public Health, or related field.2-3 years of administrative and/or supervisory/managerial experience Knowledge of best practice working with children and families in health, preventive and wellness care Bilingual a plusCMCS offers:Medical, Dental & Vision Insurance403(b) Retirement PlanGenerous Paid Time OffFlexible Work SchedulesMake a Difference Every DayIf you're ready to make a meaningful impact and be part of a compassionate, dedicated team, apply today and start your journey with CMCS!#HR24S
