Licensed Practical Nurse -Home Care Weekends

Overview Sentara Home Care Services is seeking a qualified Licensed Practical Nurse (LPN) to join our team in Virginia Beach, VA as a full‑time weekend day shift provider. The position supports patient care under the supervision of an RN or Licensed Independent Practitioner and involves direct patient care, medication administration, patient education, and collaboration with interdisciplinary team members. Responsibilities Provide direct patient care and nursing interventions in hospitals, nursing homes, assisted living facilities, outpatient clinics, or private homes. Perform medication administration, testing, and implementation of nursing care plans. Serve as a patient advocate, promoting illness prevention and holistic patient care. Collaborate effectively with physicians, nurse practitioners, and physician assistants to support clinical care. Communicate patient information and clinical updates to team members and family. Qualifications Trade school graduate (required). Active LPN license in Virginia. BLS certification within 90 days of hire. Minimum 2 years of clinical experience (required).
