Work alongside an experienced care team in a collaborative, resident-focused environment dedicated to excellence in care.Summary of Responsibilities:The Licensed Practical Nurse (LPN) delivers person-centered nursing care by administering medications and treatments, monitoring resident well-being, and fostering an environment that supports independence, dignity, and quality of life. This role works under the supervision of a Registered Nurse. This role requires direct and indirect clinical care, oversight of Certified Nurse Aides (CNAs), coordination with interdisciplinary teams, and active participation in safety, infection control, and quality assurance initiatives within the skilled nursing and rehabilitation departments.Essential Functions/Duties:Clinical Care and DocumentationAdminister and document medications, treatments, and diagnostic tests accurately, following physician orders, facility policy, and all relevant regulations.Monitor for and promptly report adverse reactions, medication effectiveness, and inappropriate prescriptions to the RN or physician.Transcribe medication orders with precision and maintain current, complete, and compliant medication records.Perform nursing assessments, routine evaluations, and ongoing documentation of resident well-being, including physical, cognitive, and behavioral changes.Demonstrate thorough knowledge of medications, including identification, usage, side effects, and proper modes of administration.Maintain medication carts, storage areas, and medication refrigerators in compliance with policy; ensure expired or discontinued medications are removed and disposed of properly.Oversight and Support of Certified Nurse Aides (CNAs)Supervise and support CNAs to ensure delivery of quality, person-centered care aligned with care plans and facility standards.Monitor CNA documentation for accuracy and completeness; provide real-time coaching and feedback to ensure compliance with care protocols.Serve as a clinical resource and team lead, assisting CNAs as needed and modeling best practices in resident care, teamwork, and communication.Resident-Centered Engagement and SupportEncourage and support resident independence in daily living activities, adapting care approaches to promote dignity and individual capabilities.Utilize appropriate interventions for residents with cognitive impairments, such as dementia, incorporating strategies that respect the residents' autonomy and safety.Foster meaningful, compassionate, and respectful relationships with residents, families, and team members.Environmental Safety and Infection ControlMaintain a clean, safe, organized, and pleasant environment in all resident care areas.Proactively identify and report equipment hazards or unsafe conditions; follow appropriate protocols for safety and emergency preparedness.Comply with infection control standards, including hand hygiene, use of personal protective equipment, and prevention of cross-contamination.Participate in regular safety and emergency response drills to support community preparedness.Communication and Interdisciplinary CollaborationMaintain open, timely, and professional communication with residents, families, and the care team.Report significant observations, incidents, or changes in condition promptly to the charge nurse or physician.Actively participate in shift change reports, care conferences, and interdisciplinary team meetings to ensure coordinated care planning.Flexibility and ProfessionalismAdapt to changing resident needs, team dynamics, and departmental priorities with a positive and solution-oriented mindset.Willingly assume additional responsibilities as assigned by management to ensure continuity of care and optimal team function.Maintain a professional demeanor and contribute to a culture of kindness, respect, and continuous improvement.Regulatory Compliance, Training, and Continuous ImprovementStay informed of and compliant with all relevant local, state, and federal regulations, as well as internal policies and procedures.Promote and uphold the organization's core values, customer service expectations, and commitment to resident safety and excellence in care.Participate in and complete all required ongoing training, including monthly online and in-person training, annual skills competencies education, and role-specific certifications, to ensure continued competence and adherence to organizational and regulatory standards.Required Skills/Abilities:Possess outstanding customer service abilityAbility to maintain accurate records and produce effective documentationExcellent oral communication skills including a clear speaking voiceAbility to work under conditions that may be distractingAbility to handle multiple interruptions of routine tasksExcellent organizational skillsAbility to adapt to changing needsAbility to use a resident and customer-focused approach to problem-solving and resident careExcellent interpersonal skills and professional demeanorAbility to effectively communicateSupervisory Responsibilities:The Licensed Practical Nurse provides direct supervision and oversight to Certified Nurse Aides (CNAs), ensuring quality care delivery, accurate documentation, and adherence to care protocols.
