Overview

Our mission is to help frail seniors maintain independence and dignity by remaining in their own homes. Through the PACE (Program of All-Inclusive Care for the Elderly) model, we provide a comprehensive, patient-centered alternative to nursing home care. This program delivers coordinated healthcare and social support through an interdisciplinary team, improving quality of life for eligible seniors.

We are expanding and looking for driven healthcare sales professionals to grow awareness and enrollment within the community.

Position Summary

The Healthcare Sales Representative (Outreach Specialist) is responsible for developing and managing referral relationships, educating community partners about PACE services, and achieving monthly enrollment goals within an assigned territory. This field-based role is ideal for self-motivated individuals with experience in post-acute care, senior services, or managed care environments.

Working under the direction of the Regional Sales Director, you will drive referral generation and enrollment growth across professional, community, and government channels.

Key Responsibilities

B2B - Sales & Business Development

• Execute territory sales and marketing plans to meet or exceed enrollment targets (6-8 enrollments/month after ramp-up)
• Develop and implement strategic outreach plans for PACE services within assigned territory
• Prospect and identify new referral opportunities through research and networking
• Build and maintain relationships with referral sources, including:
  • Healthcare providers and facilities (inpatient/outpatient)
  • Independent living communities
  • Government agencies
  • Faith-based and community organizations
• Increase referral volume through proactive account management and follow-up
• Meet activity benchmarks:
  • 2 self-generated leads per day with at least 35% conversion to qualified referrals
  • 25 sales call attempts per week, with at least 10 meaningful contacts
• Deliver effective presentations and proposals to referral partners

B2C - Participant Engagement

• Present program benefits to prospective participants and families
• Manage and qualify leads from call centers, brokers, and internal teams
• Identify and engage eligible populations (Medicaid, Medicare, Dual Eligible, VA, and Private Pay)

CRM & Reporting

• Maintain accurate, real-time records of all sales activities in CRM (Salesforce preferred)
• Track outreach efforts including calls, meetings, follow-ups, and results
• Manage referral accounts through consistent communication and relationship building
• Align customer needs with program benefits using consultative selling techniques
• Assist in developing and delivering educational programs for partners and internal teams
• Provide regular reporting on performance and goal attainment
• Perform additional duties as needed to support team operations

Qualifications

• Proven experience in healthcare sales, business development, or related field
• Background in post-acute care, senior services, or managed care preferred
• Strong communication, presentation, and relationship-building skills
• Self-starter with the ability to manage a territory independently
• Experience using CRM systems (Salesforce preferred)
• Passion for improving access to care for seniors
