Key Responsibilities
- Evaluate and treat residents within a Continuing Care Retirement Community (CCRC) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to support resident outcomes
- Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards
Required Qualifications
- Active Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program
- Experience providing physical therapy services in a skilled nursing, CCRC, or long-term care environment
- Ability to work day shift hours
Preferred Experience / Skills
- Prior experience in a CCRC or senior living community setting
- Familiarity with geriatric patient populations and related therapy protocols
