Job Title Physical Therapist Location South Carolina Schedule / Job Type Contract | Days | Onsite Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans and goals focused on progressing patients toward discharge within an established length of stay Collaborate with the interdisciplinary team to address all barriers to discharge, including equipment trialing and ordering, patient and family training, home exercise program issuance, and planning for continuing services Complete medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines Manage individual, concurrent, and group treatment sessions effectively based on patient care needs, demonstrating flexibility in all aspects of caseload management Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements Required Qualifications Active Physical Therapist license in good standing in the state of South Carolina Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans Proficiency in medical record documentation in compliance with state, federal, and payer guidelines Commitment to patient and employee safety standards and safe patient handling practices Preferred Experience / Skills Experience in an acute rehabilitation or inpatient hospital setting Familiarity with interdisciplinary team collaboration and discharge planning processes Experience supervising or mentoring therapy students Active involvement in continuing education and professional development activities Why Join Us Opportunity to work in a patient-centered environment that prioritizes safety, quality care, and professional growth Supportive setting that encourages individualized professional development planning and active participation in continuing education Concentric Healthcare Staffing Job ID 788635.
