Job Description:

The Clinical Care Coordinator, Social Worker, will manage acute inpatient medical case management and discharge planning for complex patients. The role requires working independently and efficiently within a large academic medical center environment.

Responsibilities:

• Manage a caseload of 15-20 patients per day with complex discharge planning needs.
• Consult with RT and Pharmacy for discharge planning as needed.
• Participate in department-specific orientation and collaborate with the case management team.

Requirements:

• Active LMSW or LCSW and MS State License.
• Minimum 2 years of acute inpatient medical case management experience in a large, busy hospital.
• Recent acute care discharge planning experience is required.
• Proficiency in EPIC charting and strong computer skills.
• BLS Certification.
