divh2Registered Nurse/h2pThe Registered Nurse maintains a current PA Nursing License as an RN and has experience working in acute care, long-term care, or with individuals with intellectual/developmental disabilities. In this role, you will assist with the individuals in the Residential and Community Base Services program, with coordinating healthcare services for individuals and participate as a key part of the interdisciplinary team. In caring for patients, you will develop and implement healthcare goals, communicate with various staff members who support your patients, monitor medication and equipment usage, identify ethical issues, keep an eye out for hygiene and injury concerns, and maintain electronic health records./ppThe Program Nurse must work well in a team environment, have a compassionate demeanor, use excellent communication skills, have an eye for detail, and adapt quickly to changing circumstances./ph3Basic Qualifications/h3pEducation/Training Current PA Nursing License, either RN. Diploma, Associates, or Bachelors Degree in Nursing or a related field preferred./ppExperience Two years of experience in an acute or long-term care facility or working with individuals with intellectual or developmental disabilities is preferred. health of individuals served./liliDevelops and implements healthcare goals based on needs./liliMaintains electronic health records for assigned individuals./liliPrepares a 90-day nursing assessment of individuals in the Older Adult program./liliCommunicates pertinent medical information to appropriate persons, including the program coordinator, families, and day program staff./liliMonitors the hygiene of individuals served and initiates corrective action as needed./liliAttends specialty medical appointments as needed./liliProvide health-related training to agency staff and members of the interdisciplinary team./liliEnsures completion of medical appointments./liliCompletes pre- and post-appointment paperwork./liliObtains and transcribes physician orders./liliMonitors medication distribution, storage, and disposal./liliCoordinates ordering of medical equipment and supplies./liliCompletes all required assessments, medical histories, health risk screening tools, and monthly/routine nursing notes./liliAssumes responsibility and accountability for own clinical judgment and actions./liliTakes an active role in identifying ethical issues that impact care and advocates resolution./liliAssists in identifying and obtaining appropriate medical care./liliProvides coverage for nurse colleagues as needed./liliParticipates in the nursing on-call and clinic
