Responsibilities

• Evaluate and treat short-term and long-term residents in the SNF setting
• Deliver occupational therapy services with the supervision and assistance of COTAs; complete timely documentation and treatments
• Develop individualized treatment plans based on physician orders to improve independence in ADLs, functional mobility, cognition, upper extremity function, and overall quality of life
• Collaborate with an interdisciplinary team to promote safe discharge and maximize each resident’s functional potential through various treatment approaches

Requirements

• Active Ohio Occupational Therapist license
• Ability to provide OT services in a Skilled Nursing Facility setting
• Availability for 8-hour shifts, five days per week (flexible)
• Ability to complete timely documentation and treatment plans
