You will manage complex cases and provide high-level care that keeps our clients safe and healthy at home. Primary Accountabilities Clinical Oversight: Initiate and manage new cases, conduct home evaluations, and provide clinical supervision for our team of CNAs. Complex Nursing Care: Administer physician-prescribed medications, perform advanced wound care, and manage medical devices. Patient Advocacy: Accompany and advocate for clients at physician appointments and lead hospital discharge planning for seamless transitions. Education & Empowerment: Provide essential teaching to patients and families to support informed decision-making and self-care. Documentation: Utilize EMR software to communicate clinical updates to the DON, ADON, and families in real-time. Who You Are We are looking for a Servant Leader with deep clinical roots and a commitment to professionalism. Required Qualifications Nursing Background: 5+ years of RN experience in Rehab, Long-Term Care, or Assisted Living. Case Management: 5+ years of experience in client care planning and management. Credentials: Active Rhode Island RN License, CPR Certification, valid driver's license, and an insured vehicle. Critical Thinking: The ability to rapidly assess symptoms and make high-stakes decisions in a home environment.
