A healthcare facility is seeking a licensed Pharmacy Clinical Pharmacist with hospital inpatient experience to support a long-term acute care (LTAC) setting for a 13-week travel assignment. The position requires a diligent professional capable of maintaining medication safety and supply, ensuring regulatory compliance, and providing clinical expertise.

Responsibilities:

• Manage pharmaceutical inventory, including ordering, storage, and removal of outdated or recalled medications
• Prepare, label, and dispense medications, including sterile and parenteral products, following strict protocols
• Maintain accurate documentation and records in compliance with federal and state pharmacy laws
• Conduct regular inspections of patient care areas and pharmacy to ensure proper medication handling and safety
• Oversee controlled substances, ensure accountability, and maintain documentation for regulatory compliance
• Provide medication education and discharge counseling to patients and families
• Participate in antimicrobial stewardship by interpreting culture results and recommending treatment plans
• Monitor patient drug therapies for interactions, contraindications, and adverse reactions
• Support pharmacy staff training, quality assurance, and policy development
• Utilize EPIC system for medication reconciliation and billing functions
• Collaborate with clinical teams and participate in Pharmacy and Therapeutics Committee activities

Qualifications and Desired Experience:

• Current pharmacist licensure with prior hospital or LTAC experience required
• Basic Life Support (BLS) certification
• Proven clinical knowledge in antimicrobial stewardship and pharmacokinetic dosing management
• Proficiency with EPIC or similar electronic health record systems
• Strong communication skills for interacting with patients, families, and healthcare providers
