Licensed Vocational Nurse (LVN)

Job Summary

Encinitas Post-Acute is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our nursing team. The LVN is responsible for providing quality nursing care to residents in a skilled nursing facility in accordance with physician orders, established care plans, facility policies, and applicable state and federal regulations.

The ideal candidate is committed to resident safety, quality care, and teamwork while helping maintain the comfort and well-being of our residents.

Responsibilities

• Conduct initial and ongoing assessments of residents' health status.

• Monitor vital signs, administer prescribed medications, and observe changes in residents' conditions.

• Report significant changes in residents' health status to the Registered Nurse (RN) or appropriate healthcare provider.

• Administer medications according to physician orders and facility policies, ensuring accurate dosage, proper route, and timely administration.

• Monitor and document residents' responses to medications and treatments.

• Provide basic wound care and other treatments as ordered.

• Monitor wound healing and report complications or concerns.

• Collaborate with Registered Nurses, Nurse Managers, Certified Nursing Assistants (CNAs), and other healthcare professionals to coordinate resident care.

• Participate in care planning and contribute to individualized resident care plans.

• Maintain accurate and timely nursing documentation.

• Follow infection control protocols, safety procedures, and resident rights requirements.

• Comply with applicable state and federal regulations and facility policies.

• Help maintain a safe, clean, and comfortable environment for residents.

Qualifications

• Nursing education that meets California LVN licensure requirements.

• Current, active, and unrestricted California Licensed Vocational Nurse (LVN) license.

• Strong communication, organizational, and clinical documentation skills.

• Ability to recognize changes in residents' conditions and respond appropriately.

• Ability to work collaboratively with the nursing team and other healthcare professionals.

• Commitment to providing compassionate, resident-centered care.
