This Home Health Physical Therapist Assistant role is based in Portsmouth, New Hampshire, a coastal Seacoast city known for its historic charm, working waterfront, and vibrant downtown. Your work will have a direct impact on helping patients remain safely in their preferred environment while reducing hospitalizations and enhancing overall quality of life. Typical responsibilities include implementing therapeutic exercises and functional mobility training, educating patients and caregivers on home exercise programs, safe transfers and gait, fall prevention strategies, and energy conservation. You will assist with home safety recommendations, adaptive equipment training, and modification of interventions based on patient response, reporting changes in condition to the PT and interdisciplinary team. Documentation is completed in the field through an electronic medical record, allowing for timely, accurate charting and communication. You can expect a varied but manageable caseload with visits scheduled across a defined territory to minimize drive time. A typical day includes traveling between patient homes, providing 30–60 minute visits, and collaborating closely with the supervising PT, nursing, occupational therapy, and other home health team members. Patient populations commonly include older adults managing chronic conditions, individuals recovering from orthopedic surgeries, patients with neurologic diagnoses, and those transitioning home from hospitals or rehab facilities. Shifts generally follow daytime hours with some flexibility to accommodate patient needs, making this setting ideal for clinicians who value autonomy, predictable schedules, and meaningful one-on-one care. Weekend or limited on-call rotations may be required depending on agency needs, but the focus remains on providing comprehensive, patient-centered care in a supportive environment.
