Physical Therapist
Seven Healthcare are seeking an experienced Physical Therapist for a contracted role in Beckley, WV. This is an exciting opportunity to work onsite providing vital physical therapy services based on physician referrals. You'll utilize your expertise in Home Health and OASIS within a supportive and professional environment.

Key responsibilities of the Physical Therapist include:
- Perform physical therapy evaluation and treatment based on physician orders/referrals.
- Complete comprehensive evaluations using appropriate tools and procedures.
- Review patient records for current diagnosis, medical history, and precautions.
- Establish measurable goals and develop treatment plans for patient independence.
- Re-assess treatment plan effectiveness and modify as needed.
- Communicate patient progress and needs to the treatment team and family.
- Consult with providers regarding evaluation, treatment, and patient discharge.
- Recommend and assist in obtaining adaptive equipment to enhance rehab programs.
- Maintain compliance with documentation and professional practice standards.
- Supervise Physical Therapist Assistants and Aides.
- Participate in staff development programs and continue clinical education.
Requirements of the Physical Therapist include:
- WV PT license (Can accept compact licenses)
- Experience with OASIS and Home Health
- HCHB preferred
