Home Health RN - Registered Nurse

Job Title Registered Nurse – RN Office Location: Cookeville, TN Job Type: PRN Overview We are looking for an RN to join our Home Health team and make a difference in the Mt. Juliet area and surrounding communities. Responsibilities Provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinate the total plan of care with other health‑care professionals and maintain continuity of patient care by communicating with physicians, agency personnel, patients, families, and community resources. Conduct initial or comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state guidelines, and ensure patients meet home‑health eligibility and medical‑necessity guidelines set by the payer. Maintain and submit accurate documentation and clinical notes, including patient’s condition, appropriate goals, interventions, and patient’s response to home‑health care. Provide high‑quality clinical services within the scope of practice defined by state nursing laws, from admission through discharge. Work on the road, commuting reliably to multiple patient residences each day within the geographic area assigned. Qualifications One (1) year of clinical RN experience. Current RN license in the state where services are rendered. Current CPR certification. Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation. Ability to commute reliably to multiple patient residences daily within the geographic area assigned.
