Overview Pediatric Behavioral Health Integration Coordinator Department: CHS Primary Pediatrics Clinic Location: Golisano Children's Hospital, US:NY:Buffalo Work Type: Full-Time Scheduled Work Hours: 8AM to 4PM, 9AM to 5PM, 10AM to 6PM Job Description The Pediatric Behavioral Health Integration Coordinator is a core member of the collaborative team, including the patient\'s medical director, and psychiatric consultant, as well as the larger primary care team. The Coordinator is responsible for supporting and coordinating the mental health and physical health care of patients on an assigned patient caseload with the patient\'s medical provider, and when appropriate, other mental health providers. Responsibilities Support and coordinate mental health and physical health care for assigned patients in collaboration with the patient\'s medical and mental health care team. Serve as a liaison among the patient, medical director, psychiatric consultant, and primary care team to ensure integrated care planning and follow-up. Coordinate referrals, treatment planning, and communication across services to support patient outcomes. Education and Credentials Master\'s degree in Social Work, Psychology, or Mental Health Counseling required. Master-level licensure candidate/trainee (e.g., LMSW) preferred. Licensed Mental Health Counselor preferred. Licensed Social Worker preferred. Master Social Worker (LMSW) within 18 months of hire required. Experience 6 months experience with screening for common mental health and/or substance abuse disorder required. 6 months experience with engaging patients in therapeutic relationships when appropriate. 6 months experience with assessment and treatment planning for common mental health disorders. 6 months experience with knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders. 6 months experience of basic knowledge of psychopharmacology for common mental health disorders within appropriate scope of practice.
