PRN Physical Therapist (PT) – In-House Rehab
Aventura at Shiloh Springs – Trotwood, OH

Looking for a flexible PRN opportunity with a team that values great therapy and great teamwork? Aventura at Shiloh Springs is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team !

This is a great opportunity for a PT who enjoys working with the skilled nursing and long-term care population and wants the flexibility of a PRN schedule while making a meaningful difference in residents' lives.

What You'll Do

• Evaluate residents and develop individualized physical therapy plans of care

• Provide skilled PT services focused on mobility, strength, balance, safety, and functional independence

• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team

• Provide resident and family education as appropriate

• Complete timely and accurate documentation

• Support safe discharge planning and continuity of care

What We're Looking For

• Graduate of an accredited Physical Therapy program

• Current Ohio Physical Therapist license in good standing

• SNF/LTC experience preferred

• Experience with Net Health and PointClickCare (PCC) preferred

• Strong communication and teamwork skills

• Dependable, compassionate, and resident-focused
