Licensed Vocational Nurse
We are recruiting for a Licensed Vocational Nurse to join our growing team. A UHC PACE LVN shows excellent customer service skills using genuine warmth with patients and has the ability to make them feel comfortable in the health center while providing effective and technically competent nursing assistance to both participants and UHC providers. The LVN in examination and treatment of patients under the direction of a physician. This position is a crucial part of the service delivery team responsible to ensure quality of care, patient flow and compliance with regulations within the scope of their respective nursing license. Primary tasks are to assist the Health Center Manager at the site. LVN's may be required to draw and collect blood samples from patients and prepare specimens for laboratory analysis. Finally, the LVN position provides or facilitates the provision of information needed by both patients and clinicians. Furthermore, they ensure a smooth and well-connected patient flow. The LVN reports to the Health Center Supervisor and Director.
Responsibilities
Works closely with the Health Center Manager and other members of the clinic management team to:
- Under the supervision of the Health Center Manager will provide support for medical staff
- Supports the quality improvement and compliance goals of the health center by ensuring that UHC PACE meets required regulations.
- Review physician orders, lab results and care instructions with patients, answering questions and administering treatment under the scope of their prospective nursing license.
- Communicates with appropriate personnel as necessary, to examine, revise and evaluate inner-clinic processes to promote efficiency
- In-services provider and nursing staff as needed regarding procedures, forms, and procedural changes, etc.
- Provides oversight for clinic medical support staff in the absence of the Health Center Manager.
- Provides prompt medical support; promotes a smooth patient flow; collects and records data accurately; maintains order of exam rooms, equipment and supplies
- Provides basic education and information to participants, making sure participant's questions are answered and anxieties alleviated and interfaces with community-oriented activities and staff
- Consistently and openly communicates with the Health Center Manager and all PACE center staff.
- Works flexible or extended hours where necessary.
- Must be able to travel.
Performance Area 1: Nursing Duties
- Supports patient access by screening walk-in patients and routes appropriately.
- Meets regulatory and continuity of care by utilizing standing orders and established screening guides under supervision of physicians.
- Supports participant access by providing telephone advice regarding care per health center protocol.
- Supports the efforts of the providers by supervising the functions in treatment room and is primary in this regard.
- Supports participant care by assisting the physician and/or provider in procedures and pelvic exams
- Supports participant care by administering medication and treatment ordered by physicians and/or provider staff.
Performance Area 2: Focus on Participants
- Listens attentively to participants' complaints and record all information (e.g. medication, vitals, blood sugar levels) on chart or in electronic health record system.
- Consistently secures and protects participant information: keeps paper files out of reach of participants/public. Where active, takes steps to ensure electronic health information is kept confidential. This is done by activating screen saver, minimizing screen when leaving the area, or log off when walking away from computers for any period of time.
- Helps ensure that exam rooms are neat, clean and set up appropriately for each patient exam.
- Performs specimen collection, preparation and maintains required logs.
- Performs EKG's, accu-checks, and other diagnostic procedures according to guidelines.
- Independently performs urinary catheter placement and IV starts with appropriate RN or Physician oversight.
- Supports the participant experience by taking initiative to keep participant informed of upcoming procedures and requirements, facilitates participant focus, involvement, and cooperation.
- Provides participants with required educational materials including; vaccine information statements, informs participants of content and answers any questions.
- Routes all formal participant complaints/grievances to Health Center Manager. Solves what problems they can at "point of contact."
Performance Area 3: Focus on Clinicians
- Works in conjunction with the Medical Reception staff for walk-in participant (i.e., obtains and documents participant chief complaint and presents to provider to determine if participant is to be seen or needs to be referred out).
- Helps the providers maintain pace by keeping them informed of participants that are ready.
- Prepares and sets up participants for exams according to providers' expectations.
- Alerts clinicians of possible complicated visits so they are prepared.
- Assists providers when assistance is needed.
- Helps participants by providing basic knowledge of types of services provided by outside facilities.
Performance Area 4: General Corporate Expectations
- Attends and actively participates in all meetings (e.g., department meetings, program meetings, employee staff meetings) and other activities as required or assigned.
- Attends workshops/seminars as necessary to increase skills and knowledge to provide effective care, treatment, and/or leadership.
- Supports the overall needs of the PACE center by working flexible or extended hours when necessary.
- Supports the needs of the health center by traveling to other UHC clinics when staffing needs dictate the need for personnel.
- Demonstrates awareness of, and compliance with, organizational mission and objective of UHC to provide health care access and support services for all members of the community.
- Supports their own staff development by completing the required hours of continuing education each year.
- Other work-related duties as assigned by supervisor. Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally either verbally or in writing.
- Maintains confidentiality and respect for information regarding patients and other team members; abides by UHC Rules of Confidentiality and general HIPAA regulations regarding privacy.
- Displays a positive, professional and respectful demeanor at all times toward employees, peers, professional contacts, and participants served, maintaining a professional appearance and positive image for the health centers.
- Contributes to the team by promoting positive staff interaction, maintains open communication throughout PACE center staff.
Qualifications
License/Certification:
- Valid LVN License
- Possess a valid Driver's License
- Possess a valid Advanced Cardiovascular Life Support (ACLS) Certification
- Valid IV Certification
Prior Experience:
- Minimum 1 years of experience in a healthcare setting, preferably in an ambulatory care setting
- Experience with IV starts and Catheter
Skills:
- Bilingual (English/Spanish) preferred, but not required.
- Able to quickly build and maintain rapport with participants and providers of differing backgrounds; team player
- Knowledge of physical, mental and social needs of frail older adults.

The hourly wage range that UHC reasonably expects to pay for the position upon hire starts at $29.28 an hour. Our salaries are dependent on knowledge, skills, and experience.
