Overview Multi-site role between Idylwood Care Center (Sunnyvale) & Santa Clara Post Acute (Santa Clara). Position: Physical Therapist. Work: In facility. Reporting to Director of Rehab. Key Responsibilities You assess patient needs, develop recovery plans, deliver physical therapy, and train patients and caregivers on skills that promote independence. You collaborate with peers, make referrals, and adapt to patient needs. Qualifications Valid Physical Therapy license in state(s) of practice or proof of eligibility. Master's degree in Physical Therapy (or related field) or Bachelor's with 5 years of progressively responsible PT experience. Degree earned outside the U.S. must be evaluated as U.S. equivalent.
