• Setting: Large teaching hospital (500+ beds)
• Contract Length: 13 weeks
• Shift: Day shift, Monday - Friday (Case Management RN / RN Case Manager)
• Shift Hours: 7:30am - 6:00pm
• Hours per Week: 40
• Shifts per Week: 5
• Estimated Weekly Pay: $2,072 - $2,302
• Start Date: June 7, 2026

Job Requirements (Registered Nurse / RN - Case Management)

• Active RN license in Massachusetts (or compact state eligibility) - Massachusetts RN required for hire
• At least 1 year of nursing experience required
• Minimum 6 months of experience in a large organization or teaching hospital (500+ beds)
• Recent experience in Case Management, care coordination, or discharge planning preferred
• Ability to provide two professional references
• Completion of a skills checklist required
• SSN and date of birth required for onboarding
• License verification required

Responsibilities - Case Management RN / RN Case Manager

• Coordinate patient care and discharge planning in collaboration with interdisciplinary teams (physicians, social work, rehab, and outpatient providers)
• Assess, plan, implement, and evaluate individualized care plans for patients to ensure safe transitions of care
• Advocate for patient needs and ensure continuity of care throughout the hospital stay
• Facilitate clear communication between patients, families, and healthcare providers to support optimal outcomes
• Maintain accurate documentation and comply with hospital policies and regulatory requirements for case management
