About This Role As a Cardiology RN at Yale New Haven Health, you will be an integral member of the healthcare team, providing high-quality patient-centered care while upholding the organization's mission, vision, and values. Highlights Location: Fairfield, CT What You'll Do Establish and maintain therapeutic relationships with patients and families. Communicate and collaborate with patients, families, and healthcare team members. Respect patients' values, preferences, and expressed needs. Collect and analyze pertinent health data for patient assessments. Develop and modify individualized care plans. Coordinate care delivery and transitions in care. Promote health and a safe environment through health teaching. Evaluate progress toward health outcomes. Requirements Education Graduation from an accredited School of Nursing. Experience Foundational job description for all RNs. Licenses & Certifications Licensed and currently registered nurse in the State of employment. #J-18808-Ljbffr
