Physical Therapist PRN

Job TitleThis individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client's home/residence.Essential FunctionsEvaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting.Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.Develops an individualized plan of care involving the client, specific to the needs identified while collaborating with the physician.Communicates with physician any requests to revise the plan of care.Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.Position QualificationsGraduate of a university approved by American Physical Therapy AssociationLicensed in the state of Iowa to practice physical therapyMinimum of 1-year experience preferredCommunity or home health experience desirableOur CommitmentRooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
