We are seeking a compassionate and organized Social Services Assistant to join our team at Highland Park Rehabilitation & Care. As a Social Services Assistant, you will play a vital role in ensuring our residents receive exceptional care and support. Responsibilities:

Provide emotional support and companionship to residents, addressing their social, emotional, and spiritual needs
Coordinate with interdisciplinary teams to develop and implement individualized care plans
Facilitate resident engagement in activities, outings, and events, promoting socialization and overall well-being
Assist with daily living tasks, such as personal care, grooming, and meal assistance
Maintain accurate and detailed records of resident interactions, needs, and progress
Collaborate with family members and caregivers to ensure seamless communication and support
Participate in staff training and education programs to stay updated on best practices and regulatory requirements
