Seven Healthcare are seeking a dedicated RN - Care Manager to join their team in San Antonio, TX. This travel assignment focuses on managing complex pediatric cases in a dynamic work environment, offering an opportunity to provide expert resource management for patient care delivery.

Key Responsibilities of the RN - Care Manager
  • Provide leadership and expertise to ensure effective resource management
  • Enhance continuity of care and cost-effectiveness through case management
  • Monitor length of stay (LOS) to ensure optimal patient management
  • Collaborate with healthcare team for effective discharge planning
  • Utilize MCG/Interqual guidelines for care management and resource allocation
  • Participate in interdisciplinary rounds to formulate discharge plans
  • Adapt care methods to patient-specific needs
Requirements of the RN - Care Manager
  • Graduate of an accredited school of Nursing; BSN preferred
  • Currently licensed as RN in the State of Texas (or compact)
  • A minimum of 5 years in clinical practice for the population to be served
  • Experience with Case Management and discharge planning preferred
  • Familiarity with MCG and Interqual criteria highly preferred
  • Proficient word processing skills in a Windows environment
  • Ability to create and analyze reports; familiarity with MIDAS and Meditech preferred
