In this role, you will provide counseling, support, and case management services to patients and their families. The ideal candidate will have strong clinical skills, excellent communication, and a passion for helping individuals navigate emotional, behavioral, and social challenges. Responsibilities include:Conduct psychosocial assessments and develop individualized care plansProvide individual, group, and family counseling to patientsCollaborate with physicians, nurses, and interdisciplinary teams to support patient careConnect patients and families with community resources and support servicesDocument patient progress and maintain accurate, confidential recordsSupport crisis intervention and provide guidance in high-stress situationsAdvocate for patients' needs and assist in care planning and discharge coordinationRequirements include:Degree in Social Work (MSW) from an accredited programCurrent state license as a Licensed Clinical Social Worker (LCSW)1+ year of clinical experience in a healthcare or mental health setting (preferred)Strong counseling, assessment, and case management skillsExcellent communication and interpersonal abilitiesBLS certification may be required depending on setting
