Community Health Worker

Provide care coordination and resource navigation to assigned community-member clients with unmet social needs Conduct telephonic and face-to-face outreach, follow-up, coaching, needs assessments, education, and service enrollment assistance Administer SDOH screenings, intake forms, and assessments in the Activate Care platform Help clients prioritize goals and create client-centered care plans Coordinate with community nonprofits and resources Improve clients’ health literacy and self-sufficiency through resource provision Coordinate post-hospital-discharge and post-ER healthcare transitions to support continuity of care and prevent readmissions Coordinate care across healthcare settings and facilitate specialty or behavioral-program referrals Support Medicaid and health-plan navigation, service authorizations, benefits/appeals understanding, and advocacy Provide person-centered coordination for high-risk members facing housing instability, complex medical conditions, or acute social barriers Assist with appointments, applications, reminder calls, and other assigned case activities Maintain client privacy and confidentiality Participate in weekly team meetings, workshops, and training Commute to clients’ homes and field locations as required Requirements Minimum high school diploma or equivalent Active Certified Community Health Worker I (CHW I) credential through the Nevada Certification Board, or willingness to obtain certification upon hire Valid Nevada driver’s license Personal vehicle for commuting to clients’ homes Experience working with individuals and families on social and health needs Experience creating client- or patient-centered action plans and connecting clients with services and resources Understanding of barriers to accessing community-based services and support Ability to work independently with minimal supervision and solve problems creatively Detail-oriented with focus on delivering the program model as designed Experience using electronic platforms to document care and interactions and maintain high data-collection standards Experience working with nonprofits, social service providers, faith-based groups, or government agencies addressing social determinants of health Strong independent working skills and communication Collaborative approach and commitment to supporting colleagues Cultural humility and ability to communicate effectively across diverse backgrounds and communities Ability to commute to clients’ homes and field locations The company will not sponsor applicants for work visas at this time Core Competencies Demonstrates expertise in care coordination and resource navigation for clients with unmet social needs, utilizing strong communication skills and cultural humility to connect individuals with essential services.
