Social Worker, MSW- Case Management LIFE, Full-Time, Vineland/Williamstown

MAJOR FUNCTION:Under the supervision of the Social Services Manager plans, organizes and implements social work services to INSPIRA LIFE participants and families. Responsibilities include but are not limited to assessment, treatment, teaching and counseling to participant, caregiver, or other appropriate representatives. The Social Worker Interventions could include individual participant contacts; appropriate collateral contact; participant and family education, assessment, and counseling; provision of resources; addressing mental health needs as they arise; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures. The Social Worker is the liaison between the interdisciplinary team, caregiver representatives, and community agencies. Completes NJ Choice Level of Care (LOC) Assessments initially and annually along with the required QA compliance. As a multisite provider of PACE community-based services for seniors in Cumberland, Gloucester, and Salem Counties, an Inspira LIFE employee may work at either or both the Vineland or Williamstown LIFE Program as census changes or operational needs occur to meet the needs of our participants.QUALIFICATIONS:Education & Experience:Master’s degree in social work from an accredited school of social work.2 years’ experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.Minimum of 1 year working with the frail or elderly required.Experience with Life or PACE program preferred.Certification/Licensure:Member of the Academy of Certified Social Workers (ACSW) or other NASW-recognized certification preferred.Licensed with the New Jersey Board of Social Work required.NJ State Choice Assessment Certification to complete level of care assessments– This certification can be obtained within the first 6 months of employment.Physical Requirements: N: Never O: Occasionally ( 80%)Lifting 50lbs N Crouching O Reaching OCarrying F Hearing C Walking CPushing F Talking C Vision CEnvironmental Conditions:Noise F Varied Temperatures F Cleaning Agents ONoxious odors N Patient Exposure C Operative Equipment FBENEFITS INFORMATION:Click Here to Review Our Great Benefits Offerings?>At Inspira Health, you’ll join with the area’s most dedicated and distinguished team to bring quality and compassionate care to our communities.
