A healthcare organization in Canarsie, Brooklyn, is seeking an experienced Nurse Practitioner to provide patient care through telehealth.

Location: Canarsie, Brooklyn, NY

Schedule: MondayFriday, 9:00 AM5:00 PM

Salary: $155,000$175,000 annually

Important: This is not a remote or work-from-home position. The provider must work on-site from the organization's Canarsie office while conducting patient appointments virtually.

Responsibilities

• Conduct virtual patient evaluations and follow-up visits
• Diagnose and manage acute and chronic conditions
• Review medical histories, medications, laboratory results, and diagnostic reports
• Develop treatment plans and prescribe medications when appropriate
• Provide patient education and preventive-care guidance
• Coordinate care with physicians, nurses, facility staff, and other healthcare professionals
• Complete accurate and timely documentation in the electronic medical record
• Maintain professional and efficient communication with patients and the clinical team

Requirements

• Active New York State Nurse Practitioner license
• Strong primary care or adult medicine experience
• Comfortable evaluating and treating patients through telehealth
• Strong clinical judgment and communication skills
• Ability to work independently while collaborating with a clinical team
• Comfortable using telehealth technology and electronic medical records
• Must be available to work on-site in Canarsie Monday through Friday
