Home Health Scheduling Coordinator

To support patient care coordination, the full-time Home Health Scheduling Coordinator will manage scheduling for field staff, assist with admissions and intake information, and communicate staffing needs while working remotely. Key responsibilities: Compile daily schedules for clinical staff within the HCHB workflow structure Assign POD clinicians to patients and assist in care coordination as a liaison between field staff and patients Process visit requests and communicate with the Assistant Clinical Manager regarding workflow issues Required qualifications: Associate's degree preferred At least one (1) year of experience in home health preferred At least one (1) year of customer service experience Proficiency in Microsoft Office suite Minimum of two years general office experience, including data entry or word processing
