Hospice RN Admissions Nurse

Addus Family Company is seeking a Hospice RN Admissions Nurse to lead compassionate admissions for patients entering hospice care. In this field-based role, you will conduct comprehensive nursing assessments, verify eligibility, educate patients and families on hospice services, and develop initial plans of care in collaboration with the interdisciplinary team. You’ll coordinate smooth transitions from hospitals, facilities, or homes, ensure regulatory and documentation compliance, and communicate closely with physicians.
