Registered Nurse Hospice

Job Title: Registered Nurse - Hospice Location: Bristol, NH, and surrounding towns Job Type: Full-Time Position Summary The Hospice Registered Nurse acts as the Case Manager for hospice clients, working closely with all members of the Interdisciplinary Team (IDG) to create a highly personal and individualized plan of care. They organize and direct services for clients, families, and caregivers based on identified needs and goals. Experienced in nursing with emphasis on community health and hospice, palliative care, or end of life care. Full‑time, part‑time, and per‑diem opportunities available with competitive salary and benefits. Training is provided for qualified candidates. Primary Duties and Responsibilities Patient Care Complete initial assessment of patient and family to identify immediate needs and bring them to the IDG. Complete the HIS within two days of hospice election and again at discharge, in accordance with the hospice Conditions of Participation. Complete nursing comprehensive assessment on admission and election of hospice. Coordinate with IDG to ensure all disciplines complete assessments within five days of hospice election, in accordance with the hospice Conditions of Participation. Develop a care plan with IDG that establishes goals based on diagnosis, physical, emotional, and spiritual needs. Communication Prepare clinical notes and communicate with the primary physician/hospice medical director every fourteen days. Communicate with physician regarding patient needs and report changes in condition. Obtain/receive physician orders as required. Communicate with entire interdisciplinary team to coordinate/update client’s plan of care. Ensure documentation is completed and submitted on time in compliance with the hospice Conditions of Participation. Qualifications, Skills, and Competencies Graduate of an accredited school of nursing. 1‑2 years of recent patient‑centered care/nursing experience. Current state licensure, CPR certification, and valid driver’s license. Bachelor’s degree in nursing with one year of hospice care experience preferred. Strong observation, communication, and problem‑solving skills. Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.
