Benefits and Compensation Mileage reimbursement Medical, Dental & Vision insurance 403b Retirement Plan Paid Time Off & paid holidays Continuing education and professional development Supportive interdisciplinary hospice team Flexible field-based schedule Career growth opportunities CPR and required annual education provided Requirements & Qualifications Active Connecticut RN license Graduate of an accredited nursing program (BSN preferred) Minimum 1 year of hospice experience Valid driver's license and reliable transportation CPR Certification Strong assessment, communication, and case management skills Computer proficiency with electronic medical records Must Haves Manage a hospice caseload from admission through end-of-life care Perform comprehensive hospice assessments and develop individualized care plans Coordinate care with physicians and the Hospice Interdisciplinary Group (IDG) Provide symptom management, pain control, and end-of-life nursing interventions Educate patients and families on disease progression, medications, and the dying process Coordinate services with facilities, caregivers, and community resources Complete timely hospice documentation and regulatory compliance requirements Participate in weekend/holiday on-call rotation as scheduled Preferred Background & Skills Previous hospice or palliative care experience Home Health or Visiting Nurse experience Strong end-of-life symptom management skills Medicare Hospice Conditions of Participation knowledge Case management experience Experience working with interdisciplinary care teams Excellent patient and family education skills #J-18808-Ljbffr Nbutexas
