Care Coordinator – Home Care Eastern SlopePeopleCare Health Services is hiring a Care Coordinator to manage client caseloads and coordinate quality home care services across various counties in Eastern Colorado. This role includes assessments, client follow-up, caregiver coordination, scheduling support, documentation, and communication with families, providers, and partner agencies to ensure care is delivered effectively and compassionately.What You'll DoManage an assigned caseload of clientsCoordinate care with clients, families, caregivers, and community partnersComplete intake assessments and required client visitsSupport caregiver scheduling and coverage needsHelp address client or staff concerns promptly and professionallyWhat We're Looking ForHealthcare, case management, or caregiving experience preferredStrong communication, organization, and problem-solving skillsComfortable working independently, in a fast-paced environment, and as part of a teamWork Setting This role includes working from home, attending occasional in-office meetings, visiting client homes, and traveling throughout the field.
