Description

• Conduct comprehensive evaluations of patients in both inpatient and outpatient settings and tailor treatment plans accordingly
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
• Educate patients and their families regarding therapeutic goals, discharge procedures, and home exercise regimens
• Maintain accurate documentation of evaluations, treatment plans, and patient progress in accordance with all facility and regulatory guidelines
• Engage actively in care team discussions and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure required for Physical Therapist
• Preferred: CPR certification
• Specialized certifications (e.g., neurological or geriatric) are advantageous but not required
