Family Nurse Practitioner

Team members join a community-oriented practice committed to inclusive, culturally responsive care. Role Description This is a full-time, on-site Family Nurse Practitioner role based in Phoenix, AZ. The Family Nurse Practitioner will provide primary care services to pediatric, adolescent, and adult patients, including health assessments, diagnosis, treatment, and follow-up care. Daily responsibilities include conducting patient exams, ordering and interpreting diagnostic tests, prescribing medications, and managing acute and chronic conditions. The role also involves providing patient and family education, coordinating care with other clinicians, and supporting urgent care needs as appropriate. The practitioner will document clinical encounters accurately, follow evidence-based guidelines, and contribute to a collaborative, patient-centered environment across the clinic locations. Qualifications Demonstrated proficiency in Family Nurse Practitioner practice, including Nurse Practitioners and Nursing skills. Experience in Primary Care, providing comprehensive, ongoing care across the lifespan. Strong clinical background in Medicine, including assessment, diagnosis, and treatment planning. Ability to manage Urgent Care needs, including triage, stabilization, and appropriate referrals. Current, unrestricted Nurse Practitioner license in Arizona and national certification (FNP or equivalent). Graduate degree from an accredited Nurse Practitioner program. Excellent communication and interpersonal skills, with the ability to work effectively in a bilingual and multicultural setting. Commitment to evidence-based practice, patient safety, and collaborative care with physicians, nurses, and support staff. CPR/BLS certification; pediatric experience is strongly preferred. #J-18808-Ljbffr
