This is a 14-week contract assignment with possible extension. If you meet the qualifications listed below and are interested, please apply now!Location: Bennington, VT 05201Duration: 14-week contract with possible extensionWork Schedule: Evenings (3:00 PM – 11:30 PM) Guaranteed Hours: 40 Hours/WeekResponsibilities for this position include, but are not limited to:Administer medications and treatments as prescribed by the physicianMonitor and assess residents' health conditions and report changes to the RN or physicianProvide direct nursing care, including wound care, catheter care, and other clinical proceduresMaintain accurate and timely patient documentationAssist residents with activities of daily living (ADLs) as neededCollaborate with physicians, RNs, CNAs, and other healthcare professionals to ensure quality patient careFollow infection control policies, safety standards, and facility guidelinesDeliver compassionate, resident-centered care while promoting dignity, comfort, and independenceTo be considered for this position, you should have:Minimum 2 years of Long-Term Care (LTC) experience preferredExperience in Skilled Nursing Facility (SNF) settings preferredStrong medication administration and documentation skillsExcellent communication and patient care abilitiesAbility to work independently and as part of a multidisciplinary teamGraduate of an accredited Practical Nursing (LPN) programActive Vermont LPN License (or Compact License, if applicable)BLS Certification requiredTryfacta is an Equal Opportunity/Affirmative Action Employer.
